Forms needed for your
annual
registration:
Registration
Form
-- Everything you need to re-register your scout for the year.
Health
and
Medical
Record -- Each scout must have this form on file before
he can participate in an outing. The form remains valid for
12 months, so it must be re-filed every year.

Here are some forms that you
may need:
Service Hours
Credit Form -- For reporting non-eagle community service that
you have performed. (You must have the Scout Master sign this before
you perform your service.)
Reimbursement
Form -- Use this form to request payment or reimbursement from
the treasurer. You must request reimbursement within 90 days
of the expense.
For the parents of first-year
scouts:
When you are new to Boy Scouts, it's hard to
know how to help your son have a postive experience. Here is some good
news: we have prepared Tips for
a Successful
FirstYear, which explains some of the
scouting terms you are starting to hear, and describes some of the
things you can do to help make your son's (and your own)
scouting experience positive, fun, and memorable.
Outing permission slips and
reporting:
Tour Permit Form
-- For organizing trips and campouts. See also some guidance on how to
fill this out.
Registered leaders can submit tour permits on line: sign in to myscouting.org, then go to Unit
Tools, and then Tour Permits.
Scouts must have
written permission from a parent or guardian prior to
attending any Troop 948 outing or activity. Outing planners may choose
to have each parent fill out and sign the individual permission slip or may
have
all
parents
sign
the
group
permission
slip.
Individual
Permission Slip -- Each parent or guardian will complete one of
these forms (here's an example of one
that that is ready to pass out to the parents)
Group
Permission Slip -- One form to hold the signatures of all parents
and guardians
Medication Information Form -- Required
when a scout or adult brings any kind of medication to camp.
Activity reporting
template -- This is an Excel form for listing who participated in
an outing and the costs for the outing. ASMs should fill this
in and send it to the Advancement Chair and the Treasurer
(updated May 2009).

Opportunities
for adult leader training
The BSA's Guide to Safe Scouting for adult leaders. There's a
click-able
version for on-line reading and the whole
thing for printing or reading offline. We also have a list of training our adult
leaders have taken. (You could generate this list yourself using
out TroopMaster software.)
The Twin Valley District
provides a schedule of training for leaders. Everyone involved with the troop’s
activities, either as an ASM or on the committee should go through New Leader Essentials
training. NLE provides a basic background for all scout leaders. Youth Protection
is required for anyone who has contact with the boys, and Boy Scout Leader Basic
Training is highly recommended for anyone involved in
the troop’s activities. Training is available both on-line and in
person.
The council calendar
and training calendar
are provided by
the SF Bay Area Council. The first
shows the dates for meetings, camps, and other scout events. The second
lists
training available for adults.
